The Level 4 Certificate in First Response Emergency Care (FREC 4) is an advanced, regulated pre-hospital care qualification designed for learners who have already completed the Level 3 First Response Emergency Care (FREC 3) and want to develop their emergency medical skills further. This qualification broadens your scope of practice in pre-hospital care and supports career progression into roles such as emergency care assistant, intermediate ambulance practitioner, event medic, associate practitioner, and other specialist response positions both in civilian and emergency services settings. It follows the recognised pre-hospital emergency care pathway and meets the competencies set out in the PHEM framework at descriptor level E.
Advanced patient assessment and clinical decision-making
Physiological observations and clinical monitoring
Major incident management and triage
Airway management including supraglottic airways and oxygen therapy
Use of medical gases (including dosages and safe administration)
Trauma care including chest and thoracic trauma
Full body and limb immobilisation
ECG application and monitoring
Emergency care for sick and injured children
Mental health crisis response
Sepsis recognition and initial management
Care considerations including maternity and paediatric emergencies
Psychosocial development and context of emergencies
Qualification: Level 4 Certificate in First Response Emergency Care (RQF) – recognised pre-hospital care qualification
Prerequisite: Must hold a valid Level 3 First Response Emergency Care (FREC 3) certificate to enrol
Duration: Typically around 5 days classroom training with additional guided self-directed study
Framework: PHEM descriptor level E (expanded emergency care competencies)
Certificate Validity: Remains valid provided professional development (CPD/continuing training) is maintained
Career Pathway: Supports progression into intermediate ambulance roles, emergency care assistant positions, and specialist responder roles
